1. Two (2) valid government IDs or Passport (scanned copy)
2. Digital copy of Cash Card
3. Any of the following:
   • One month payslip from employer with signature (scanned copy)
   • Proof of employment (contract from Principal Employer) with POEA stamp (scanned copy)
   • Optional: POEA or OWWA  license of employer (If Pag-IBIG requests authentication of Contract with POEA Stamp) (scanned copy)
4. Any of the following:
   • Certificate of Net Pay from employer with signature at the back (scanned copy)
   • Certificate of Remittance or ESAV (optional) (scanned copy)

Unfortunately, seafarers are not covered by the DOLE CAMP P5,000 subsidy. Seafarers have an option to avail the DOLE subsidy program Tulong Panghanapbuhay sa Ating Displaced/Disadvantaged Workers Program (TUPAD).
